OBSERVATORY: CITIZENS PROMOTE hospital care

Emergency nurses and the deficit in health system national, but the citizens are satisfied and remain beyond the critical care hospital. Osservasalute Data Report 2010, presented yesterday

ROME - The public is satisfied with the hospital, while the remaining emergency nurses and the deficit in the National Health System. These are only some of the results of the eighth edition of the Report Osservasalute (2010) presented today. These results, especially in health in Italy.

EMERGENCY NURSES - Although nationally there has been a sharp increase between 2005 and 2007 the number of units both in absolute terms (+4.6%) and in relation to the population to 1,000 residents (+3.2% ) for some time now we talk emergency nursing from the lack of nurses, hospitals and community services are in crisis. In this regard, it is stressed that, if between 2005 and 2006 there was an increase in nursing staff employee of the NHS between 2006 and 2007 the number of units in both absolute and per 1,000 inhabitants decreased.

Hospitalisation rate - In Italian hospitals admissions continue to decline and in parallel reduces the days in hospital. From 2001 to 2008 the crude rate of hospitalization of the national total was reduced by about 22 percentage points (from 214.6 to 192.8 per 1,000 inhabitants). The reduction in this period is attributable to one component of inpatient admissions increased from 161.4 per thousand to 133.3 per thousand, being, however, increased slightly during the period, albeit discontinuous, the proportion of admissions under the daytime (from 53.2% to 59%). In this regard it is noted that in 2008, all regions still have a discharge rate (Td) for the day shelters above the standard reference of 36 admissions per 1,000 inhabitants (20% of Dh with respect to a general set of 180 Td per thousand). In 2008 the standardized rate of hospitalization overall national is 187.3 per thousand, of which 129.1 per thousand in ordinary mode and 58.2 per thousand in day care arrangements. The overall hospitalization rate varied regionally, the maximum value present in Ontario (231.8 per thousand) to the minimum value of the Friuli Venezia Giulia (148.5 per thousand). Even last year we analyzed the ratio shows a steady reduction in rates of hospitalization. To reduce the overall rate is 3.0% in 2008 compared to 2007, 8, 0% compared to 2006. For the mode of treatment in the ordinary scheme is the reduction of 2.9% compared to 2007 and by 6.9% compared to 2006 for day care is a reduction of 3.3% over 2007 and 10.3% compared to 2006. The rate of the RO system ranges from 100.3% to 160.1 per thousand in Tuscany Puglia. All regions have a rate of hospitalization for DH than the reference legislation by 36 per thousand. The shelters are the DH in 31.1% of the total. With regard to the hospitalization rate regime daytime regions with extreme values are the Liguria (88.8 per thousand) and Friuli Venezia Giulia (37.3 per thousand).

public health spending - the trends already recorded in previous editions of the Report: the current public health expenditure in relation to the GDP growth at national complaint, from 2001 to 2007, from 5.95% to 6.59%. At the regional level of the indicator, in 2007, shows significant differences, ranging from a minimum of 4.90% in Lombardy to a maximum of 10.76% of Calabria, recording, thus, a gap that is nearly the 6 percentage points. We observed a north-south gradient, with the north recording a percentage of public health expenditure to GDP than the average current equal to 5.45%, 6.25%, the Centre and the South (South and Islands) equal to 9 57%.

per capita health expenditure - The Report shows a national increase in spending per capita between 2008 and 2009, from 1782 € to 1816 € (+1.91%), spending has grown by 27.8% since 2003. In 2009 and observe that, generally, the southern regions, with the exception of Molise, provide an amount of monetary resources below that of the North (subject to Lombardy and Veneto) and the national average. In 2009, spending citizen has a national average of € 1,816 and ranges from a minimum, recorded in Sicily, of € 1,671 to a maximum of 2,170 € in the Autonomous Province of Bolzano. Compared to 2008, all regions, with the exception of the Autonomous Province of Bolzano (-2.78%) and Lazio (-0.35%), increased the level of expenditure, with values that mark a growing line that starts from a low of 0.28% of the Abruzzi, to a maximum of 5.3% of the Autonomous Province of Trento.

DEFICIT - In 2009 the National Health Service confirmed a total of a deficit of € 3.260000000000, amounting to € 54 per capita. It should be noted, however, that this is the deficit more than 'lowest since 2004, highlighting a trend of steady, albeit slow, decrease. Remain, the strong regional differences, with a gradient more and more 'wide between the North and Center-South, home to most of the deficit. In particular, there are eight regions that are confirmed in equilibrium since 2007: Piedmont and Emilia-Romagna (which, however, 'worked' coverage of interventions against their regional budgets) Autonomous Province of Bolzano and Friuli Venezia Giulia ( which provides special status, however, 'a particular system of funding); Lombardy, Tuscany, Umbria and Marche. Register a deficit of € 21 per capita, the Veneto, although even in this case has been used to load additional resources the regional budget. As for the regions subject to repayment plans, placing will continue in 2010 because the three years originally planned, has proven inadequate to balance the management. In 2009, only Sicily (46 €) and Abruzzo (37 euro) are positioned below the national average per capita deficit, while Lazio and Molise regions confirmed the 'deficit is for 2009 (respectively 244 € and 225 € per capita) is given in the cumulative 2001 - 2009 (€ 1803 and € 2,285 respectively). Finally, a clear deterioration of economic and financial situation is recorded in Calabria, in 2009 subject to the repayment plan: its deficit increased from € 33 per capita in 2008 to € 111 in 2009, also due to the absence of relevant coverage Budget Regional.

PROMOTED BY CITIZENS hospital care - Italians are quite satisfied with care received during hospitalization: results of a survey conducted by the Centre, in collaboration with ISTAT, show the overall positive opinion on ' assistance received during the hospital admission. The survey, which covers the period 2007-2009, was performed on a sample of about 24 000 families (a total of about 54 000 individuals), distributed over about 850 Italian municipalities of different population size. At the national level as much as 36.1% of people emerging from a shelter is very satisfied receipt of medical care and 52.8% fairly satisfied, the citizens which indicated "very dissatisfied" do not reach the 4% and indicated their "dissatisfaction" is 7.8%. It follows a north-south gradient that penalizes the southern regions whose satisfaction levels are always lower than the national average.

"In summary, the picture that appears from the report this year shows various signs of improvement on the side of some risk factors," said the Director of Osservasalute Walter Ricciardi, "but not enough to reverse the trend of a general country firmly on the road a difficult future, especially in some regions, the combination of an aging population, increased risk factors, particularly behavior in young people, and regional administrative structures in trouble in providing services, especially prevention and early diagnosis can reduce the disease burden. Of particular concern are some areas still without organs, structures and activities of programming, on the one hand allow to properly analyze the needs and demand for services by the target population and, second, to provide for human resources, financial, logistical and technological capabilities to meet these needs.
